This occurs when you are logging in for the first time on a machine/user account and the database authentication information has not yet been configured. It can also occur if browser cookies have been deleted.
When you are presented with a screen asking to configure a server, username, password, and database, you must enter the database authentication information, not your actual username and password for URTO/UW2.
That information is as follows:
Select SQL Server Authentication.
Once you have input this information and click OK, you should be presented with the standard login.
